When you come for your first visit, please remember to bring the following

• Driver’s License or a valid ID
• Insurance information
• Referral Letter (if required)
• Previously completed imaging studies such as X-rays, MRI, CT scan etc. Please include both images and radiologist reports if available.
• List of medications (if any)
